New Driver License Documentation Requirements
Starting January 1, 2010, when you go to a driver license office, you must bring original documents that prove your:


  1. Identity




  1. Social Security Number




  1. Residential Address

The checklist contains some valid documents. For more information, go to www.GatherGoGet.com. If you do not have Internet Access, telephone (850)-617-3995.


1. Identification: US Citizens (BRING ONE) - Bring one of the following original documents as proof of your identity: Certified U.S. birth certificate, including territories and District of Columbia (a hospital birth certificate is not acceptable).


  • Valid U.S. Passport




  • Certification of naturalization






2. Social Security Number (BRING ONE) - Bring one of the following original documents as proof of your Social Security number, if issued:





  • W-2 form




  • Pay stub showing number




  • SSA-1099




  • Non-SSA 1099 (See definition of acceptable Non-SSA Variants of Form 1099 below)



3. Residential Address (BRING TWO) - The address on a driver license or ID card can no longer be a post office box. Bring two proofs of residential address.





  • Florida votor registration card







  • W-2 Form or 1099 form




  • Utility bill, hookup, or work order not more than 2 months old




  • Current automobile or homeowner's insurance policy or bill







  • Medical or health card with address listed







  • Educational transcripts for current school year




  • Statement from an accompanying parent, stepparent or guardian residing at the same address as applicant. The parent or guardian must present proof of residential address.

Non-SSA Variants for Form 1099







  • 1099-B: Proceeds from Broker and Barter exchange Transactions




  • 1099-C: Cancellation of Debt







  • 1099-DIV: Dividends and Distributions




  • 1099-G: Government Payments







  • 1099-INT: Interest Income




  • 1099-LTC: Long Term Care Benefits







  • 1099-OID: Original Issue Discount




  • 1099-PATR: Taxable Distributions Received from Cooperatives







  • 1099-R: Distributions from Pensions, Annuities, Retirement plans, IRAs, or Insurance Contracts




  • 1099-S: Proceeds from Real Estate Transactions
